The ones in the U.S. appear to be Indian reservations. I'm not surprised, seeing as many Indian tribes respect "Two-Spirits", an umbrella term for LGBT persons. India and Thailand also have cultures of MtF transgenderism, called Aravani girls and Lady-boys, respectively. The Philippines also celebrates a third MtF identity, called Fa' fafine.
