That not how the underworld usually works. In ancient myths there were three distinct areas of said underworld. The first is Elysium, or Elysia in some works, which is home to heroes such as Theseus and Jason it could be equated to heaven. The second one is the Pits of Tartarus where those are punished are placed, people like Tantalus, Sisyphus, and John Wayne Gacy would be here, also we would equate this to Hell. The third is the Fields of Asphodel, a grey place where the majority of souls end up and more than likely this is where Ethan is. We would probably equate this with Purgatory.
