So much in one chapter. The main thing is that David is taking charge and has some clarity about what he wants and what needs to be done to accomplish that. Now if he can just get his dad and grandad on the same page....
So it's not all sweetness and light for David...or Mark. That's hardly surprising but it does raise some more speculation and questions.

Our hospitals and doctors are great at dealing with physical wounds. The invisible wounds to the psyche are much harder to diagnose, treat, or cure and can be intractable.
While most people have heard of PTSD, most people don't know that there can be a delayed onset, sometimes months or years after the initial traumatic episode.
Carina, Jett and Theo/April all with an interest in Jay who is enjoying being the center of attention. I think things could be easier to sort out than the protagonists think. That's my guess and I'm sticking to it.
